I confirm we have met with Sheri Cappello or Melinda Placanica for a Meet & Greet, and we would like to start the process to participate in the program.
The SLC is a community based program dedicated to supporting individual academic and social goals. We are here to foster learning and social engagement.
Be advised, SLC does not employ aides or medical personnel and cannot provide medical or physical assistance, including but not limited to first aid, administering medication, or assisting with personal care or daily activities. In the event of a medical emergency, we will contact you immediately and call 911.

Once we receive this "I Choose Success" form you will be sent an invoice for the $150 Application fee. This is a one time only expense which encompasses our initial consultation, classroom observation and student processing into the program. The invoice will be due upon receipt and is not covered through alternate payment options such as Self Direction.

Within seven days you will be contacted by our Director of Student and Family Services, Annette Earl. Annette will discuss your individual needs, calculate a yearly estimate and review your payment options.

Students will be unable to register for classes until this process is complete, so please read and respond to emails and keep in touch during the process. ​Class sizes are limited, and semester by semester some locations may have limited seats available.
