Transportation Waiver and 30 minute pick up

By submitting the waiver below Student, along with Student’s parent or legal guardian (each a ”Guardian”) attending classes, workshops, lectures, clubs, or any Successful Learning Center, Inc. (“SLC”) scheduled events, at any SLC scheduled location (“Premises”), acknowledges and agrees to the following:

Student shall either travel to and from class by himself or herself, or a family member, guardian or aide shall transport Student to and from class. SLC shall have no responsibility to transport Student.

SLC doors open at 9:45am and close at 2:45pm. Students are not permitted in the facilities unless Extended Mentorship is prearranged. Early drop off or late pickup will be invoiced monthly.

SLC shall have no responsibility for Student if Student is (a) dropped off at the Premises more than 30 minutes before the scheduled class start time or (b) picked up more than 30 minutes after the scheduled class end time.

If Student is dropped off or picked up outside the periods set forth above, neither SLC nor its officers, directors, employees, contractors, shareholders and agents, nor any entity that owns, operates, manages or otherwise oversees the Premises (collectively, the “SLC Parties”) shall be liable for any claims, damages, injuries, actions or omissions of any nature suffered by Student or anyone claiming through Student.

Student and Guardian hereby waive any claim against the SLC Parties, and Student and Guardian hereby release the SLC Parties from any claims, damages, injuries, actions or omissions of any nature that may arise out of the subject matter of this Waiver Student and Guardian hereby agree to indemnify, defend and hold harmless the SLC Parties for any such claims arising out of or related to the subject matter of this Waiver, including but not limited to any claims brought by any person or entity claiming through Student or Guardian.

If Student is a minor and less than the legal age of consent or is otherwise unable to legally consent to these Guidelines, Guardian warrants that the undersigned parent or guardian has legal custody of Student and the capacity to execute these Guidelines on Student’s behalf.

In case of emergency or illness, the SLC requires the contact information for a designated individual responsible to pick up a student on campus or in the community within 30 minutes of our call. If this contact is for a group home or residence, please provide a secondary number. Phone line must be able to receive text messages.
